Pediatric Dental Practice of Dr. Viena Posada - Basic Registration Information

This is basic information we would like to review with you:

Please update information regarding:

Changes in medical history

Life events that may be affecting your child

Change of address and telephone numbers

Changes in dental insurance

Your cell phone number or alternate phone numbers where you can be reached

 

Scheduling of Appointments:

Children under the age of 8 will be scheduled in the morning most of the time

Certain procedures such as extractions or restorative work requiring anesthesia or Nitrous Oxide will be scheduled in the morning.

We understand the desire to avoid children missing school and have structured our schedule to provide optimal treatment in a time-efficient manner. Afternoon appointments are reserved for older children and those undergoing orthopedic or orthodontic treatment, since their appointments are so frequent -- usually every 4 to 5 weeks.

Please call in advance if you are unable to be present for your appointment. We would like to offer that time to patients on our waiting list, whenever possible.

 

Records: Please feel free to ask for a copy of your child's records at any time

Billing:

We are pleased to bill your dental insurance carrier as a courtesy to you.  You are responsible for any co-insurance, co-payment or deductible that may be applicable.

We do not routinely bill. Instead, we will collect your estimated balance at the time of your appointment.

If you have insurance, please review the services that are covered during the year since changes regularly occur.

Non-covered services:  most insurances only cover one treatment of fluoride a year.  The American Dental Association recommends a professional fluoride treatment every 6 months for a moderate risk patient, and every 3 to 6 months for a high risk patient. Please let us know if this procedure can be done as the ADA recommends, given your insurance plan.

White fillings: Some insurances base the costs on silver fillings. White fillings being slightly more costly, you will be responsible for the difference not covered by insurance.
